Request For Documentation Form

An RQD enables birders to document unusual records for possible inclusion in the Minnesota Ornithologists' Union permanent file. Your submitted form will be evaluated by our Records Committee. If the record is significant, submitted details may be published in a future issue of our journal, The Loon.

The Request for Documentation form (RQD) can be viewed as an Adobe PDF file and printed directly from your screen. Instructions for use are included on the form, as is an address where you can send the completed report.

The RTF version of the RQD should be readable by most any standard word processor.

Seasonal Report Form

The Seasonal Report form enables birders to contribute their sightings to "The Season", a quarterly summary of Minnesota observations. The Seasonal Report form can be viewed as an Adobe PDF file and printed directly from your screen. Instructions for use are included on the form, as is an address where you can send the completed report.

Youth Mentorship Form

The MOU has a Youth Mentorship Program (YMP) with many volunteers. A YMP volunteer visits schools or other organizations and gives a presentation with slides of birds and takes the group on a field trip, all at no charge. See our Youth Mentorship page for more information.

Minnesota Nest Card

Nest cards are used to report observations of nesting birds in Minnesota. Results of breeding bird information gathered in this way are published in the Summer Season of our journal, The Loon. The Nest Card can be viewed as an Adobe PDF file and printed directly from your screen. There are four cards per sheet. Instructions for use are included. Send completed cards to the Summer Seasonal Report compiler.

Brood cards are used to report observations of recently hatched birds in Minnesota. Results of breeding bird information gathered in this way are published in the Summer Season of our journal, The Loon. Birds of Minnesota Checklist

Also available is a complete Minnesota checklist. Separated into groups representing Regular, Casual and Accidental species, this checklist fits on a single side of an 8.5" X 11" sheet of paper with 1/2" margins.
